fix(ui): attribute panel incidents and clarify account lines (#203)

Attribute panel incident badges to the affected provider's meter icon with a standalone fallback when no meter can badge, keep the badge visible during refreshes, keep the vertical identity icon badging only its own provider's incident (foreign incidents keep the standalone dot), place the plan beside the account email with working elision on narrow popups, and stop presenting operational status or repeated provider names as overview identity.
