fix(notifications): ignore failed status fetches as incident evidence (#609)

A returned unknown severity is missing status evidence, even when the status object exists. Preserve prior incident state, avoid false network-failure alerts and wait for an authoritative startup baseline while quota observations still run.

Documentation: update usage, development and Unreleased; TODO parity entries and README behavior summary are unchanged.
