fix(usage): preserve quotas with malformed plan metadata (#243)

Build the Codex plan label from the validated login method already selected for account identity. Reading the raw metadata again could throw during formatting, discarding healthy quotas and named accounts, or suppress a valid fallback.

Add production-parser regression coverage and retain independent exception-containment checks. Update the usage guide and Unreleased. TODO, CLI contracts, configuration, translations, packaging, and README remain unchanged.
