feat(costs): mark totals that exclude incomplete requests (#282)

Official CLI 0.60.5 reports `incompleteRequestCount` for requests that
lacked final usage and were therefore excluded from the tokens and cost it
publishes. The widget showed those totals as complete measurements.

Normalize the count into the existing cost trust metadata, bound it like the
coverage counts, and let it qualify a displayed cost or token total. The
shared cost-trust notice now names how many requests were excluded, for a
provider and for the aggregate spend total. Keep the notice key semantic so a
changed count does not revive a dismissed warning, and keep the count out of
every amount. Privacy mode retains the count, which carries no identity.

Documentation impact: usage and cost-history guides describe the new marker
and its bounds, the changelog records the user-visible addition, and TODO
keeps the remaining per-row marking scope.
