fix(cost): keep healthy costs when error flag is falsy (#358)

costRecordHasError treated any present non-null error key as failure,
so a record with error:false dropped its totals and reported the
provider failed. Only truthy errors (and non-blank strings) now count
as failure, matching the strict provider-snapshot check.
```
