fix(cost): show refresh errors beside retained provider costs (#240)

A cached cost snapshot suppressed the provider error label after a failed scan. Keep the bounded, privacy-projected error visible with collapsed or expanded details and reuse the existing partial-history translation.

Add a real-widget smoke regression for failed replies, retained data, privacy, initial failures, and recovery. Update usage, cost, development, and Unreleased documentation. TODO and CLI contracts are unaffected; this repairs existing frontend behavior.
