fix(usage): preserve healthy records after malformed fallback siblings (#236)

A normalization exception escaped the provider-scoped record loop, so a
malformed account before a healthy one replaced valid quotas with an error.
Contain normalization failures per record and continue selecting the first
healthy snapshot for the requested provider. Keep all-invalid replies as
scoped errors and complete the request exactly once.

Extend the production-parser regression test with record order, measured
zero, provider identity, and completion assertions. Update Unreleased and
the usage/development guides. TODO, CLI contracts, configuration, catalogs,
metadata, and packaging are unaffected.
