* feat(providers): review CodexBar 0.66.0 and bundle its new providers

Official CodexBar 0.66.0 adds DevPass, Atlas Cloud, Vercel AI Gateway,
and llmman, bringing the registry to 84 records. Bundle a name, an
original non-vendor icon, the upstream dashboard, and the docs link for
each, plus brand colors for all but Vercel AI Gateway, whose white mark
would vanish on a light theme.

Probes against the checksum-verified official Linux asset, run in an
empty temporary home with synthetic keys, show that the supported
`set-api-key` writer feeds all four default sources, so they join the
API-key setup allowlist. A loopback llmman fixture verifies its memory
lane, detail rows, and optional bearer key; the lane now reads "Memory"
instead of "Session", and DevPass lanes use the upstream "Plan credits"
and "Premium weekly" labels.

The review also verifies that 0.66.0 keeps user provider plugin entries
across `config enable`/`disable`/`set-api-key`, which 0.65.0 deleted on
every Plasma toggle.

Documentation impact: adds and indexes the dated 0.66.0 review, moves
the TODO baseline to 0.66.0, rechecks the carried-forward blockers, adds
a CLI lane-label fallback entry, updates the registry count in README
and the usage guide, and warns user-plugin users to update the CLI.
