fix(providers): preserve setup state and keyboard focus (#530)

Invalidate stale diagnostics after successful writes, retain known setup capabilities during reloads, and preserve row focus without accepting repeated pending toggles. Keep API-key prompt enablement and write errors consistent.

Documentation impact: update provider behavior in docs/usage.md and CHANGELOG Unreleased. TODO CLI blockers remain unchanged; no listed parity item is completed.
