refactor(providers): extract cost and credit presentation (#454)

Move provider cost sections, reset-credit counts and normalized Codex monthly-credit rows into a localized QML component with an explicit number format. Preserve the existing pure classification and root adapters.

Document ownership and update direct component, six-catalog wiring and UI boundary checks. Regenerate gettext source references. No user-visible change or completed parity item; usage guides, CHANGELOG and TODO remain accurate.
