docs(updates): clarify when the install offer disappears (#490)

Correct the lifecycle description found while reviewing the last 20 commits. Successful installation clears the offer immediately; restart reconciliation handles updates installed another way. Failed attempts retain retry availability.

Documentation impact: update usage guide and existing Unreleased entry. TODO and CLI contracts are unaffected; no runtime behavior changes.
```
