feat(sessions): keep session state current and tidy session cards (#375)

While the Sessions tab is visible it now rescans every 30 seconds, or at the
refresh interval when that is shorter, matching the macOS local session
monitor. The CLI marks a session active only within a two-minute activity
window, so the previous refresh-interval cadence (five minutes by default)
could show "Active" beside "4 minutes ago" and keep ended sessions listed.

Session cards mark the active state with the theme's positive color instead
of the provider accent, dim idle sessions, keep a single copy action beside
the title, and drop the details copy action. Details use a middle dot
separator, name OMP Pi-family sessions from the CLI dialect, and show the
host only when sessions come from more than one host. CopyableValue rounds
its text width up so layout rounding cannot elide a title that fits.

Sessions capture and tour GIF sessions frame. Catalogs drop the unused
"Copy session details" string. TODO.md is unaffected.
