fix(providers): hide empty Codex extra usage placeholder (#421)

CLI 0.67.0 reports providerCost {used: 0, limit: 0, currencyCode:
"Credits", period: "Extra usage"} for every Codex account without a
monthly credit cap. The popup rendered it as "Extra usage: Credits 0.00".
The macOS app (v0.71.0) hides this generic fallback when used and limit
are both zero; purchased credits already appear in the Credits row.

Docs: changelog and usage guide describe the hidden placeholder.
