Use the settings secondary text tier and reveal icon labels on keyboard focus. Make truncated provider details readable on hover through the plain-text tooltip adapter.

Update usage guidance and Unreleased notes. TODO remains unchanged: no listed parity entry or official CLI contract is implemented by this presentation-only change.
