fix(providers): complete Claude Platform fallback metadata (#528)

Match official CodexBar 0.76.0 metadata while reusing the shared Claude artwork and preserving Linux cookie setup boundaries.

Update the offline 104-provider registry, public-interface regressions, translation template and five catalogs. Reconcile README, Usage, Unreleased and the completed metadata TODO; authentication and output-verification blockers remain open.
