fix(accounts): preserve healthy duplicate account quotas (#420)

Account deduplication retained the first CLI record unconditionally, so an earlier failed record hid a later successful quota read. Preserve the first healthy record at the original account position and keep distinct validated selection keys.

Add public-response and real AccountsController subprocess regressions, and remove redundant source assertions after confirming behavioral tests catch identity and prototype-lookup mutations. Update the usage guide and Unreleased changelog. TODO parity work, configuration, translations, metadata, and CLI contracts are unchanged.
