refactor(accounts): isolate account discovery (#255)

Move account processes, retained lists, and errors into AccountsController. Share bounded provider snapshots with usage refreshes while keeping selection, localization, and effects in QML owners.

Documentation: update development ownership and verification guidance. Supported behavior, configuration, CLI contracts, and TODO parity scope are unchanged.
