Refactor branch toolbar actions to use transition-based git ops

- replace React Query mutations with direct git API calls wrapped in `useTransition`
- show toast errors for branch create/checkout failures and keep optimistic branch updates
- disable branch controls during pending actions and refresh branches when opening menu
