Improve mobile chat layout and add responsive sidebar primitives

- add reusable `ui/sidebar` and `ui/tooltip` components for responsive navigation
- optimize chat header/composer spacing and mobile-safe touch targets
- add sidebar trigger in chat header and hide desktop-only controls on small screens
- make session hydration reconnects silent to reduce unnecessary connecting state flicker
