Polish auth UX and add Account settings section

- Auth incident prompt is now a ShadDialog ("Cloud connection lost") with
  plain-language copy and Reconnect/Sign Out/Not Now actions, replacing the
  Material AlertDialog that exposed Convex/Supabase jargon
- All auth error strings humanized (incorrect credentials, unconfirmed
  email, rate limits, network) instead of raw backend exception text
- Auth dialog: animated inline message banner (destructive for errors,
  info-styled for the email-confirmation notice), per-field error borders,
  labeled busy state on the submit button
- Discord OAuth no longer pops the dialog blind: a pending banner explains
  the browser round-trip and the dialog closes itself when the session lands
- Sign-out now confirms with a toast
- New Account section in app-wide settings: avatar, display name, email,
  live cloud connection status with Reconnect on incident, Sign Out / Log In
