Decode Oodle archives past 12 MiB of output

A user's 13.08 replay failed with "a token region at output 12648448 is
not supported": Selkie refused every token region from 0xbfffff on, the
limit of the spec it was written from. The spec's new "13.08 extension"
(icarus-replay-work/oodle/spec.md, written clean-room from black-box oracle
checks) fills that in. In a region starting at or beyond 0xbfffff, a far
entry whose first three bytes read 0xc00000 or more has a fourth byte worth
0x400000 each; a far count of 0xfff is replaced by the LE16 after the count
field. All 263 recorded archives inflate byte for byte, including the four
13.08 ones (quick-1308.json) and the archive with the one four-byte entry.
