Forget a reset link once its code is gone, share an in-flight request, close only a dialog on top

- Any sign-in, sign-out or Discord start forgets the pending reset link:
  each spends or replaces the code the link needs, so a request after
  one of them goes out instead of pointing at a dead link.
- A request for the same address while one is out shares its answer,
  and the form ignores Enter while busy, so a second request can't
  replace the first one's code.
- The dialog closes itself only while it is the top route; a sign-in
  landing after it was dismissed no longer closes the page beneath.
