Surface cloud failures that previously failed silently

Verification against the live deployment exposed three silent failure
paths and two paper cuts:

- openCloudStrategy returned silently when the snapshot fetch failed,
  leaving the editor on an eternal skeleton with no feedback; it now
  throws, so opening from the library toasts and backs out, and the
  create path reports instead of opening the doc as local with no sync
- createFolder fell through to the local Hive write when the cloud call
  failed, silently creating a local folder from the cloud view; it now
  toasts and rethrows, and the folder dialog stays open for retry
- Clicking the rail avatar signed out instantly; it now confirms
- The share dialog's "Active links" badge counted revoked links

TODO.md records the verification results and remaining follow-ups
(likely convex deployment skew behind the snapshot failures).
