feat: reconcile durable offline outline layout edits

Retain layout intentions with their selected property values and ancestry. Merge independent changes, preserve competing changes for exact-image review, and retain uncertain attempts without acknowledging coincidentally converged values.

Migrate prior caches to schema 7 while preserving images, pending edits, attempts, receipts, media and sequence IDs. Share active-path traversal with paragraph reconciliation.

Native controls verify five automatic merges, five reviewed merges and four retained deletions, plus dependent text and cold reopening. Tests cover twelve writers with 144 intentions, migration, restart, uncertainty and extended SQLite fuzzing.
