fix: refresh automatic titles when moving outlines

Update page and metadata title caches in the same revision as outline coordinates. Preserve explicit titles, content identities and historical revisions.

Verified against three native OneNote controls and independent cold reopens, interruption and offline lost-reply tests, 3,692 ASan executions, full core/offline/Python suites, Clippy and both iOS targets.
