feat: publish formatted insertions as one durable intent

Add nonoverlapping UTF-16 styles to paragraph and outline insertion without a second revision or offline queue entry. Preserve intent identities through serialization, replay and placement review; reject malformed spans before source parsing.

Validate native cold reads, controlled reverse edits and direct keyboard editing, empty-style continuation, historical preservation, interrupted publication, old-cache compatibility, twelve-client mixed editing and offline recovery. Retain public native fixtures and stateful sanitizer coverage.
