feat: edit outline layout and saved expansion without rewriting content

Add typed position, width and saved collapse edits with a typed user-width reader flag. Preserve identities, content, unknown properties and history, and reject protected or ambiguous ancestry before publication.

Cold OneNote captures verify five layout/default edits and replacement of an existing native wrapping reservation. Tests cover 768 stateful changes, publication interruptions, 12-client mixed layout/paragraph fuzzing, and core/offline regressions.
