feat: every command and context-menu action has an icon

New Page, New Section and the rest share the page, tab and notebook art under a
badge (add, remove, check, history, link, lock and so on), layered in art.rs.
The palette, the toolbar menus, and the page, tab, notebook, text and version
menus show them; a disabled row fades its icon, colours and all.
