feat: recordings and annotations in the page model, kept through edits

`page::Attachment::recording` names a recording OneNote captured (identity and type) and
`PageParagraph::media` a paragraph's link to a moment in recordings, what OneNote exports
as `MediaFile`, `MediaReference` and `MediaIndex`. The writer keeps both through ordinary
edits and refuses to author them, since OneNote makes them while recording; the three-way
merge carries them like tags. `corpus/media-edit` is the cold read of the native recording
page with a paragraph added in Rust: the recording and its 500 ms annotation survive.

Evidence rows: `evidence/m4/soak-restart` (twenty session launches resuming one cache
against one section), `evidence/m4/fuzz-bounded` (three minutes per fuzz target), and
`evidence/PROTECTED-WRITE-DESIGN.md` recording the protected object layout and the plan for
writing protected sections.
